scsi: libiscsi: fix shifting of DID_REQUEUE host byte

The filename of the patch is: scsi-libiscsi-fix-shifting-of-did_requeue-host-byte.patch and it can be found in the queue-4.4 subdirectory.
If you, or anyone else, feels it should not be added to the stable tree, please let stable@vger.kernel.org know about it.

The SCSI host byte should be shifted left by 16 in order to have scsi_decide_disposition() do the right thing (.i.e. requeue the command).
