I've seen this once before with v3.13-rc1-128-g0e4b074 and now this time. Note that I've only been testing on trimslice since v3.12 though (total of 87 boots). No other tegra boards seem to exhibit it.

I'm going to send patch "serial: 8250: enable UART_BUG_NOMSR for Tegra" which I believe should solve this.
